Insurance and Medical are both settings an interpreter can be cleared for, per language pair. Exams go to interpreters cleared for them.

Billing by carrier, no-shows included

Set up each carrier as an account with its own rate card. When a claimant doesn't show, cancel with the No-Show Consumer reason, and the rate card's cancellation rules decide the charge.
